This service is to look after patients who are on the serious  mental illness list in the GP practice. To complete the patients annual physical and mental health reviews.

Who can access the Serious Mental Illness HCA's service?

A patient who has a diagnoses of a serious mental health illness registered at a GP surgery in the Greater Peterborough/ Cambridge area 

What are the opening times for the Serious Mental Illness HCA's Service?

The service is available to patients Monday - Friday (09:00 - 17:00)

If I book an appointment with the Serious Mental Illness HCA's, will they have access to my medical record?

Yes, we will have access to your medical records once we have consent through your GP practice
