What we do
GPN delivers a wide range of services designed to support patients, healthcare professionals and our local health and care system.
Our work includes primary care support, community services, urgent and unplanned care, preventative health services and innovative models that enable people to receive care closer to home.
We work with our partners to improve access, develop new ways of working and make sure patients receive the right care at the right time.
Our services include:
Primary Care and Extended Access
Supporting GP practices and patients through services such as the GP Hub and other primary care programmes.
Community Services
Providing care in people’s homes and communities, including home visiting and specialist community-based services.
Urgent and Unplanned Care
Supporting patients who need urgent care and helping to provide alternatives to hospital where it is clinically appropriate.
Hospital at Home and Virtual Care
Using new models of care and technology to enable people to receive treatment and monitoring closer to home.
Preventative and Specialist Services
Supporting people to manage their health, prevent ill health and access specialist services designed around specific health needs.
