HCA Home Visiting Service

The service operates:

  • Monday to Friday: 9:00 am – 5:00 pm (excluding bank holidays)

The HCA Home Visiting Service provides vital support to Peterborough GP practices by delivering in-home care for housebound patients who are unable to attend standard primary care appointments. The service is delivered by a skilled clinical team, including Health Care Assistants, who carry out essential procedures such as phlebotomy, blood pressure monitoring, height and weight checks, and the collection of stool and urine samples. All results are fully recorded in the patient’s medical records and reviewed by the referring practice, ensuring timely and accurate follow-up care.

Many housebound patients face significant barriers to accessing primary care, including mobility challenges, chronic health conditions, and social isolation. Without in-home support, these patients may experience delays in diagnosis, treatment, and monitoring, which can contribute to poorer health outcomes and increased health inequalities. The HCA Home Visiting Service addresses these challenges by bringing care directly to patients, ensuring they receive the assessments and treatments they need in a safe and familiar environment.

For urgent needs, patients can be seen within 48 hours, allowing for timely interventions and reducing the risk of complications. The service also helps practices manage their workload by providing reliable in-home observations, enabling clinicians to make informed care decisions without requiring patients to travel.

By providing regular home visits, the service not only improves access to essential healthcare but also enhances patient comfort, promotes continuity of care, and supports the broader aim of reducing health inequalities in Peterborough.

For many patients, it ensures that routine monitoring and early intervention are possible, helping to prevent avoidable complications and hospital admissions.
The HCA Home Visiting Service plays a crucial role in maintaining the health and well-being of the city’s most vulnerable residents, supporting both patients and GP practices across Greater Peterborough.
