Housebound Diabetic Service Anonymous Patient
Our Housebound Diabetic Service provides essential diabetic reviews for patients who are unable to attend their GP practice.
Delivered by our highly trained Community Healthcare Assistants, the service enables patients to receive comprehensive diabetic care in the safety and comfort of their own home, while providing GPs with the clinical information needed to make informed treatment decisions.
Reintroduced in response to feedback from our Member Practices, the service is improving access to care, reducing barriers for housebound patients and supporting better health outcomes across Peterborough and the surrounding areas.
The Diabetic Home Visiting Service operates:
The Diabetic Home Visiting Service supports GPs across Peterborough and surrounding areas by enabling patients to receive essential diabetes care while remaining safe and comfortable in their own homes. Following each visit, GPs receive comprehensive information that allows them to make informed decisions and provide timely, personalised treatment and care plans for their patients.
Our highly trained Community Healthcare Assistants (HCAs) deliver specialist diabetic care, including annual diabetic reviews, blood tests, blood pressure monitoring, BMI assessments, foot checks, and feedback directly from patients. In addition, the team identifies and escalates any other health or social well-being concerns, ensuring a holistic approach to patient care.
The service is particularly valuable for housebound diabetic patients who may struggle to attend regular appointments at their GP practice. By bringing care into the home, the service ensures that patients receive the full range of required checks, including foot examinations, diabetic phlebotomy, blood pressure assessments, and urine sample collection for analysis. All results are securely shared with the patient’s practice, enabling the GP to complete or update treatment and care plans promptly.
By providing in-home diabetic care, the service improves access to essential monitoring, supports early detection of complications, and ensures patients remain actively engaged in managing their condition. This service not only enhances the health and well-being of housebound patients but also supports GPs in delivering high-quality, personalised care across Peterborough.
Our Health Visiting and Diabetic Health Visiting services are accessed through GP practices.
If you feel that you may benefit from the service or believe you meet the eligibility criteria, please contact your GP practice.
