Urgent Wrap Around Care

The Urgent Wrap Around Care (UWAC) service provides enhanced in-home care for acutely unwell patients, delivering a range of interventions and support aimed at preventing hospital admissions and promoting recovery in familiar surroundings. Interventions include clinical assessments, observations, blood tests and result review, wound care, ECGs, bladder scans, prescriptions, remote monitoring, and ongoing care planning.

The service accepts referrals from the East of England Ambulance Service (EEAST), Call Before Convey, community healthcare teams, and trusted assessors working within the Urgent and Emergency Care (UEC) Hub. Patients can be assessed at home within two hours of referral, ensuring timely intervention for adults registered with practices across Peterborough, Huntingdon, and Wisbech.

UWAC is delivered by a multi-disciplinary team overseen by a GP or Senior Advanced Clinical Practitioner. The visiting team includes Advanced Clinical Practitioners, Urgent Care Practitioners, Paramedics, Urgent Care Technicians, Nurses, Nursing Associates, and Health Care Assistants. The team creates individualised care plans, manages treatment for up to seven days, and makes onward referrals to other services when required. All consultations are recorded in SystmOne, with a discharge summary sent to the patient’s registered GP practice to ensure continuity of care.

By providing high-level care in the home, UWAC reduces unnecessary hospital admissions and ambulance dispatches, particularly during periods of high demand. Patients recover more quickly in familiar surroundings, maintaining connections with family and support networks, while acute care beds are reserved for those requiring hospital treatment. This approach also improves efficiency for ambulance crews, reducing time spent waiting outside emergency departments and allowing resources to be directed to patients in greatest need.

The Urgent Wrap Around Care service operates:

  • Monday to Sunday: 8:00 am – 8:00 pm, 365 days per year

Through rapid, expert assessment and intervention, UWAC supports patient recovery, relieves pressure on emergency and primary care services, and delivers safe, high-quality care in the community.

The two clinicians who visited me were very thorough and attentive. I cannot think of anything they could have done better.
